INSURANCE REQUIREMENT POLICY
Effective Date: February 26, 2026
This Insurance Requirement Policy (this “Policy”) establishes the mandatory minimum insurance requirements for all specialized telecommunications service providers (each a “Provider”) operating on the Splicer Network platform. This Policy is incorporated by reference into the Splicer Network Provider Terms and Conditions and Client Terms and Conditions. By executing a Work Order, Nevantin Services, LLC, a Texas limited liability company (“Splicer Network”), and the Provider agree to comply with the requirements set forth herein.

General Requirements. At all times while a Work Order is in effect, and for a period of six (6) months after the expiration or termination of a Work Order for any reason, the Provider shall purchase and maintain in effect, and require all of its subcontractors engaged to provide any portion of the Provider's services to provide and maintain in effect, the insurance coverages specified in this Policy. Splicer Network shall maintain comparable coverages for the benefit of its Clients.

Minimum Coverage Limits. The Provider shall maintain the following minimum insurance coverages: (a) Commercial General Liability. Commercial general liability insurance providing coverage for bodily injury, property damage, operations, independent contractors, underground explosion and collapse hazard, and contractual liability. The limits of coverage shall not be less than $1,000,000 per occurrence, $2,000,000 annual aggregate, and $2,000,000 for products and completed operations aggregate. (b) Business Automobile Liability. Business automobile liability insurance, including coverage for all owned, non-owned, leased, hired, and rented vehicles, with a combined single limit of liability of not less than $1,000,000 for personal injury, bodily injury, death, and property damage. (c) Worker's Compensation. Worker's compensation insurance for the benefit of employees with coverage of at least $1,000,000 each accident, $1,000,000 for bodily injury by accident, and $1,000,000 each employee for injury by disease, or the minimum statutory requirement in the state where the work is being performed, whichever is greater. (d) Umbrella/Excess Liability. Umbrella coverage providing liability insurance in excess of the coverage required under this Policy of at least $2,000,000. If work is performed in the Northeast, Illinois, or California, this coverage must be no less than $5,000,000. (e) Pollution Insurance. Pollution insurance coverage of at least $1,000,000 is required for all underground work performed by the Provider. (f) Professional Liability (Errors & Omissions). Professional liability insurance covering errors, omissions, or negligent acts in the performance of testing, fiber characterization, or network design services, with a minimum limit of $1,000,000 per claim and in the aggregate.

Equipment and Tools. The Provider shall secure, pay for, and maintain whatever insurance it deems necessary for protection against loss of owned or rented equipment and tools. The failure of the Provider to secure such insurance or maintain adequate levels of coverage shall not obligate Splicer Network or any of its Clients for any losses of any Provider equipment.

Insurer Qualifications. All insurance required by this Policy shall be procured from financially sound, A-rated (or A+ rated), and generally recognized responsible insurance companies authorized to sell such insurance in the state where the Provider is performing services.

Primary Coverage and Waivers. All insurance policies of the Provider shall contain an endorsement whereby the insurance carriers agree that the Provider's insurance is primary and not contributory with or in excess of any coverage which Splicer Network or its Clients may carry. The Provider is responsible for all deductible payments under its insurance policies. All insurance policies shall include a Waiver of Subrogation in favor of Splicer Network and its Clients.

Additional Insured. The Provider shall ensure that all insurance policies (except Worker's Compensation) name "Nevantin Services, LLC and its subsidiaries and affiliates," as well as the applicable Client, as additional insured parties.

Certificates of Insurance. The Provider shall furnish Splicer Network with valid Certificates of Insurance ("COI") prior to the commencement of any services. All COIs must be provided to Splicer Network directly from the Provider's insurance agent. If a COI expires, Splicer Network reserves the right to withhold any and all payments until Splicer Network receives a valid, updated COI.

Notice of Cancellation. The COI and the insurance policies required by this Policy must contain a provision stating that all coverage afforded under the policies will not be cancelled, materially altered, or allowed to expire unless Splicer Network has received at least thirty (30) days prior written notice.
